
Carea Community Health Centre (Carea) is a recognized leader in redefining the experience of community-based health and wellness services providing a variety of free community programs and services to Durham Region. Carea’s Theory of Change clearly identifies its focus on delivering integrated clinical and wellness care, equity-based programs and services, and knowledge mobilization & advocacy to address social determinants for our Durham Communities; with a strong focus on priority populations. As part of the senior leadership team, and reporting to the Chief Executive Officer, the Vice President, Integrated Care & Experience, leads Carea’s Integrated Care & Experience team that focuses on primary care, counseling services, mental health, harm reduction, chronic disease programs (COPD, diabetes, GAIN, HEP-C) and other outreach services. Carea also has a strong focus on priority populations such as 2SLGBTQI+, Black, Indigenous, New Canadians, and People of Color to ensure equity within its programs and services. The Vice President also acts as the organizations privacy officer and is responsible for agency-wide privacy related policies and procedures ensuring adherence to privacy legislation.
The position will work closely with the Chief Strategy Officer in enabling the organization to achieve an integrated care approach across Carea’s programs and services centered around a Carea community member/client. The position leads the integrated care and experience team comprised of managers, physicians, nurses, counselors, dieticians, medical secretaries, community outreach teams, and other program staff. This position is also responsible for ensuring privacy compliance for the organization.
